RENAL TRANSPLANT

Definition

Renal transplantation is a treatment method performed to eliminate dialysis dependency in patients with end-stage renal failure. It has been reported to increase patients' survival rates. Therapeutic plasmapheresis is generally used in renal transplantation for antibody-mediated rejections after transplantation, desensitization before transplantation, living donor transplants, HLA-positive cross-match transplants, transplants with high panel reactive antibodies, and cadaveric transplants.

Rationale

In antibody-mediated rejections, donor-specific antibodies are produced after transplantation. These antibodies can be removed by therapeutic plasmapheresis. Therapeutic plasmapheresis is always accompanied by immunosuppressive drug therapy. Therapeutic plasmapheresis can also be used to remove HLA antibodies before transplantation.

In antibody-mediated rejections, 5-6 sessions can be applied daily or every other day. In some protocols, the number of sessions is decided based on kidney function and antibody titers. In desensitization protocols, sessions continue daily or every other day until the patient becomes cross-match negative. In the postoperative period, 3 more sessions can be continued. Immunosuppressive drugs should be started before beginning the sessions.
